Case study:
Presentation You are caring for Michael, a 33-year-old single male who has been diagnosed with “bipolar disorder” based on his most recent hospital admission summary. His gender pronouns are he/him/his. Michael has undergone multiple trials of different medications; however, his symptoms have remained persistent despite these interventions. Over a prolonged period, you have observed chronic emotional instability, frequent episodes of intense anger, persistent insomnia, feelings of emptiness and boredom, low motivation, and diminished self-worth. Today, Michael is undergoing a diagnostic second opinion and a scheduled family meeting. The family meeting was challenging to arrange, as Michael’s parents had to travel from Malaysia to attend in person.
Diagnostic Second Opinion Meeting
During the consultation, you present clinical notes recorded by other healthcare professionals over the past two weeks. After a thorough review, the treating team concludes that Michael’s symptoms do not meet the criteria for hypomania or a major depressive episode. This raises the question of whether he was previously misdiagnosed with bipolar disorder instead of borderline personality disorder (BPD). Following this reassessment, the team decides against prescribing additional medications. Instead, they propose a gradual withdrawal of his current medications to assess their necessity while shifting the primary focus of his treatment toward psychotherapy within a community based setting.
Family Interview
Michael’s mother, Linda (55), and father, Daniel (60), are Indigenous individuals residing in rural Malaysia. They travelled for their first family meeting regarding Michael’s care. The couple has two sons: Michael (33) and Ryan (25). They disclose that Michael has been using methamphetamine, and even during periods of abstinence, his condition has not improved. Linda describes their sons as having grown up in a supportive family environment. However, Michael faced significant difficulties at school and struggled to maintain friendships. At 18, he moved out for an apprenticeship, maintaining initial contact with his parents before gradually becoming more distant. Over the past two years, Linda recalls multiple encounters with law enforcement involving Michael. She and Daniel have experienced direct consequences, including court appearances, drug-related fines, and threats from individuals linked to Michael’s criminal activities. One incident involved a car sale that resulted in phone threats from unknown individuals. At this meeting, Linda states that she has not seen Michael in person for two years. Their only communication has been through Facebook messages. She last saw him virtually during a court appearance via video link late last year.
What you are being asked to do?
Based on the information presented in the case scenario, you are expected to address the following tasks:
Task 1 – Describe the concept of recovery (personal and clinical recovery) in people living with bipolar affective disorder and borderline personality disorder. Based on the information you have found out, discuss the treating team’s second opinion decision to transfer Michael’s care to the community.
Task 2: Based on the results of the diagnostic second opinion meeting, discuss three evidence based care approaches, models of care, or frameworks of care covered in this unit that are relevant to Michael’s case. Support your response with evidence and consider Michaels’s specific presentation.
Task 3: Select the most appropriate care approach, model, or framework from those discussed in Task 3 for Michael’s care. Justify your choice with evidence from the case study and peer reviewed sources. Additionally, explain how you will engage and support Michael’s family and outline strategies to maintain his care within the community, preventing relapse.
