Assessment Instructions
The assessment instructions have been designed to enable you to understand what is required to succeed in this assignment.
Pre-amble
The term ‘complex needs’ for individuals with a mental illness is difficult to define, as it varies based on the person and their circumstances. In many cases, complex needs arise from multiple disadvantages, such as mental and physical health challenges, unemployment, substance use, and involvement with the justice system (Radu et al., 2022). These needs are typically identified through a comprehensive mental health assessment.
For this assessment, use the case scenario from Assessment 1. You will take on the role of a care coordinator, case manager, assigned mental health professional, or primary nurse.
What should you include?
▪ Task 1: Summarise the case report you completed in Assessment 1. Your summary should demonstrate that you have incorporated the feedback provided in Assessment 1. Any revisions made based on the feedback should be highlighted in yellow. The summary should include Identifying Information (please use pseudonyms), presenting the problem/issue -including the reason for the current presentation/referral, background history (Mental health and medical/surgical history, psychological history), assessment of risk, mental state examination and protective factors, drug and alcohol, and physical health assessment and functioning (~500 words).
▪ Task 2: Identify four priority areas for care related to the case scenario in Task 1. These priority areas should be directly linked to the goals of care you outlined in Assessment 1. Additionally, ensure that there is clear evidence of how you have incorporated the feedback from Assessment 1 into the goals of care section. Any revisions made based on the feedback should be highlighted in yellow (~200 words).
▪ Task 3: Create and present a care plan in any format of your choice, such as a table or paragraphs. Regardless of the format, all interventions must be supported by evidence. The goals and their evaluation should be clearly defined and directly linked to the case scenario and the priority areas of care identified in Task 3. Additionally, your care plan should specify the healthcare professionals and services involved in the patient’s care. For services, ensure that they are currently active in your country (Australia or Singapore) and include relevant web links within the care plan (~1000 words).
▪ Task 4: Record a podcast, up to a maximum of 5 minutes, presenting the care plan to a multidisciplinary team. Your presentation should also include recommendations for the
client’s care. Please note that an audio recording of the podcast will need to be submitted in addition to text document (this section is equivalent to 600 words).
▪ Include both an introduction and a conclusion (each ~100 words).
Other assessment details:
▪ Include a standard introduction that sets up the context of your report and clearly highlights what you will be discussing.
▪ Include a standard conclusion, which should provide a summary of the key points made within the assignment, reaffirming the main discussion points.
▪ References need to support your work, and recency of evidence-based practice (references) should be within the last five (5) years of publication where applicable (minimum of 10 journal articles – books and government documents can be used in addition to the 10 journal articles).
